COVID-19 SAFETY & HEALTH
Due to COVID-19, Clients and Therapist will be required to wear face masks throughout the duration of the appointment. Therapist will bring in portable equipment that has been sanitized thoroughly between visits. Therapist will also wash hands and forearms before and after each session to prevent the spread of infectious viruses and bacteria.
Clients will be scheduled over the phone. The therapist will require an intake form on each client. The intake form will request personal information such as full name, address, date of birth, medical conditions, prescriptions, etc. The discussion of a client's medical history allows adequate time for researching conditions or prescriptions to formulate a treatment plan with positive results.
Prior to each massage session, the treatment plan will be discussed with you.
There should be an appropriate amount of space to accommodate a 30”x73” Massage table.
The therapist will arrive to setup 15 minutes before each session.
It is recommended that clients wear undergarments. You will be covered with a top sheet throughout your session.
Prior to your massage, please remove contact lenses, and all jewelry. Pull long hair back with a clip or band.
Immediately after each session, the therapist packs equipment and departs within 15 minutes.
Cash/Credit only accepted.
Please provide at least 24 hours notice if you need to reschedule or cancel a massage.
The therapist is happy to adjust pressure, temperature, musical volume, work longer on an area or move on if you request it.
The client may choose to refuse any massage methods and/or stop the massage at any time.
CONFIDENTIALITY & CONVERSATION
All documentation and conversation is confidential.
The client may or may not choose to talk during the massage. The therapist is happy to listen and discuss various topics. Please refrain from topics of a political, private or sexual nature.
It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued.
The client must also keep the massage therapist informed of any changes in health conditions.
SCOPE OF PRACTICE
I am a licensed professional held to the highest standards of the American Massage Therapy Association.
Massage Therapy is a profession in which the practitioner applies manual techniques, and may apply adjunctive therapies, with the intention of positively affecting the health and well-being of the client.
Massage Therapists do not diagnose or prescribe for medical conditions nor are they allowed to provide treatment for a specific condition without a doctor’s supervision. The massage therapist is required to refer you for diagnosis and to follow recommendations of your physician.
Requests for sexual activity will not be tolerated, it will be viewed as solicitation, and reported to the proper authorities under the guidelines of the massage therapy policies and procedures. The session will be terminated immediately and the client will not be rescheduled if this occurs.
Sexual interaction or discussion of any kind between the client and the massage therapist is NEVER appropriate.
For clients residing outside a 15 mile radius from Union Dale, there will be a .50 rate per mile. Travel rates don't apply to sessions booked at Active Health Chiropractic.
